This reporting site allows Florida Hospital Employees, Medical Staff, or any member of the workplace community to report threats or matters of Workplace Violence. While anonymous reporting is an option, please be aware that should you wish to remain anonymous we cannot provide you with any follow-up to the matter you are reporting. If you desire a follow up, contact information (Email Address or Phone Number) must be provided.
This site is not intended to replace direct contact with Human Resources, Security, or your department administration or management. Instead, this site provides an avenue for reporting workplace violence incidents confidentially and securely. You may report any ethical or legal concerns to Florida Hospital Corporate Responsibility by calling 407-303-9659 or by email: FH.Corporate.Responsibility@flhosp.org. For concerns regarding an employee or employment issues, you may contact your campus Human Resources office. You may call the AHS Guideline toll-free number 24/7 at 1-888-92-GUIDE (48433). All calls are confidential and you may call anonymously.
